Inspiration
Initially, we wanted to automate certain outpatient workflows in order to save time for clinicians. However, upon digging deeper into the issue of burnout in healthcare, such an approach merely saves a bit of time, but does not tackle the root causes of burnout in the long term. We broadly classified root causes into 2 aspects: the lack of healthcare workers, and the lack of health ownership amongst the general public, especially the elderly due to literacy and information gaps.
As such, we were inspired to think through the lens of patient and caregiver empowerment, whilst minimising additional load on existing healthcare workflows.
What it does
ClearNote is an AI-powered app that translates complex medical notes into concise, patient- and caregiver-friendly summaries. After each clinic visit, users receive a simple mailbox update with visit notes, medication changes (“Start, Change, Stop”), and follow-up plans. The app also suggests reliable resources and personalized tips to support disease management at home, ensuring caregivers and patients stay informed even when they cannot be present at appointments.
How we built it
We designed ClearNote to integrate into existing outpatient workflows. Using AI summarization models, the summAIrise feature generates simple explanations of doctor’s notes. The app front-end was prototyped with caregiver and patient usability in mind — simple, clean, and mobile-first. Medication tracking was built with safety in mind, highlighting what’s new, changed, or stopped.
Challenges we ran into
Balancing accuracy with readability in AI-generated summaries. Avoiding overpromises while still making the solution impactful. Designing a workflow that integrates smoothly into clinicians’ already busy schedules. Ensuring that caregivers of varying tech literacy can use the app with ease.
Accomplishments that we're proud of
Building a solution that truly puts patients and caregivers at the center of care. Creating a clear “Start, Change, Stop” medication feature that tackles a real-world problem: medication errors. Designing an app that supports both patients and pharmacists without disrupting clinical workflows. Developing a scalable concept that can grow with healthcare systems.
What we learned
The importance of framing the problem through real caregiver stories (like Mr. Tan and his daughter). That simplicity in design and language is as critical as accuracy in healthcare solutions. How crucial it is to think about integration — new tools only work if they fit into existing systems. Caregiver empowerment directly impacts patient outcomes.
What's next for ClearNote
We aim to pilot ClearNote with outpatient clinics to validate accuracy, usability, and workflow integration. Next steps include building partnerships with healthcare institutions, enhancing language support for Singapore’s multilingual population, and expanding the AI to handle complex, multimorbid cases. Ultimately, our goal is to scale ClearNote across hospitals, pharmacies, and clinics to support patients, caregivers, and healthcare providers everywhere.
