Sun Life already has assets (coverage, claims, provider network/Lumino) that, if surfaced simply, could turn education into action.

What it does

Monitor → Prevent → Thrive learning track with bite-size modules (e.g., Know Your Numbers, Plate Power) tied to real actions.

A dashboard that shows progress, reminders, and quick links to Coverage, Make a Claim, Find a Provider—plus telehealth access.

Proactive & contingency plans: store meds, providers, labs, and emergency steps so Ruth and her caregiver always know what to do.

How we built it

PWA with modular services: Education, Tasks/Plans, Claims & Coverage, Provider Finder, Messaging.

Patient-first UX: large text, read-aloud, high contrast; SMS/phone nudges for low digital literacy.

Designed to plug into Sun Life tools and hospital workflows with minimal change.

Challenges we ran into

Converting medical jargon into plain-language micro-lessons without losing accuracy.

Balancing simplicity (one step a day) with the breadth of diabetes care.

Mapping benefits to clear next actions (e.g., “Book covered dietitian” → claim flow).

Accomplishments we’re proud of

A single Today Dashboard that unites learning progress, reminders, and Sun Life actions.

A complete “Know Your Numbers” module with baseline card and safety guidance.

Clear Desirability/Feasibility/Viability story aligned to patient, hospital, and insurer value.

What we learned

Day-0 anxiety drops when you offer one clear action and show visible progress (e.g., module chips, streaks).

Benefits only matter when they’re one tap away from the moment of motivation (inside the lesson or reminder).

Caregiver and contingency views are as critical as patient views for real-world use.

What’s next for DiabetesKit

Pilot with a Waterloo Hospital cohort: measure task adherence, benefit utilization, and urgent-care reductions.

Expand modules and templates to other chronic conditions using the same Monitor → Prevent → Thrive spine.

Deeper integrations with Sun Life (direct-pay, claim status) and provider scheduling to fully close the loop.
