The extra_attributes parameter in openssl_csr_new was implemented in a way that does not make much sense. It behaves like
dnin terms of setting subject entries but the cnf attributes are set only if this parameter is present. This changes the behavior of this parameter and sets attributes instead. It also overrides any attributes set in config. That should make the parameter more useful but it is only for master (requires updating UPGRADING) as it is in some way still BC break even though it was broken previously.
