Complete v0.4.0 release with output improvements and fixes. Changes: - Replace arrow symbol (==>) with record symbol (⏺) for section headers - Add consistent blank line spacing between sections - Reduce tool input truncation from 3 lines to 1 line - Restore line count display for truncated output (... +N lines) - Update CHANGELOG.md with v0.4.0 release notes
