This PR merges Gary's changes in from before he left to support the
application_tokensendpoint in the auth app in https://github.com/shiftcommerce/shift-authentication/pull/96.The only changes I have made relate to the realisation that we don't need
client_secretwhen exchanging anaccess_keyfor anaccess_token.
